A form letter for your broker is enclosed.

If the securities are in your possession and you have an active brokerage account, please deposit the bond(s) or stock certificate(s) with your broker and then follow the instructions as above for #2.

If the securities are in your possession and you are NOT working with a broker, please send the following items in separate envelopes:

a. The unsigned certificate along with a letter stating these securities are a gift to Grace Place for Children and Families, Inc.

b. A signed certificate release request (a copy of this form is attached)

c. A signed irrevocable stock or bond power (a copy of this form is attached)
